The said policy has been renewed from IOB Kasba Gold Park Branch, Kolkata-107, branch code 1517.policy No 2817/50120808/01/000.Date of Issue 17.06.2010While sub meeting renewal form I have enclosed the Death of my wife Padmasri Roy. Premium has been deducted from my SB A/c no 151701000003621.On 26.07.2010 I have received the Policy schedule with following Discrepancy.
1) The Policy issued on my late wife’s name.2) Premium certificate for the purpose of Income tax deduction issued on my wife’s name. Whereas the premium has been deducted from my SB a/c.
