Dear Sir,

I have been regularly receiving the Annuity Cheques against Policy Numbers 212201005 (monthly) and 212201179 (quarterly). This year, I should have received the 12 monthly cheques (Policy Numbers 212201005) by now. I am, therefore, requesting you to kindly arrange to dispatch the 12 annuity cheques (November October) at your earliest.

In case, you require the certificate of my existence please send the requisite form so that I can send you the same immediately.
