It is respectfully prayed as under:

1.) That the applicant (Amita D/o Sh. Bhajan Lal Khann) has been working as TGT (Science) in Govt. Model Senior Secondary School, Sector 44, Chandigarh.

2.) That while deducting income tax at source, the school authorities have deducted income tax amounting to Rs.7, 000/-, whereas on calculation the total income tax payable was Rs.3478/-, hence, Rs.3522/- being excess income tax was deducted, which is refundable. For ready reference, photo copy of Form No.16 is enclosed herewith.

3.) While submitting Income Tax Return for 10 (Assessment period 11), I requested for refund of Rs.3522/-being deducted excess income tax.

4.) Approximate more than a year has passed, but my refund has not been paid to the applicant.

It is, therefore, requested that either this amount of Rs.3522/-may be credited to my Salary Saving Bank Account No.5128215 1006382 (Oriental Bank of Commerce, SCO 264, Sector 44-C, Chandigarh), or a cheque/draft may be sent to the applicant at the following address.
