Dear Sir,

This is to inform & request you that on

14.6. myfather Mr M.M Sharma had taken your New Jeevan Dhara

Pan, Policy no:201095119 in which name of the propser is me ie Rahul

Issar.

However my father died on 9.2.2007 in a natural manner at his

residence in Morar, Gwalior.(X-copy of certificate attached during

filing of claim).

I learnt about this policy in year & immediately filled

&submitted the claim form at your Morar Baradari Br.

As on date more than one year has passed & no reply has been recieved

by us regarding the status. On visiting your branch the concerned

officers only say that they are finalising & as on date nothing is

recieved by us.

His earlier claims after his death for policyno's:20108671905808

&201105807were recieved promptly by us.

The usual statement made by your officer is that in death certificate

name mentioned

of my father is M.M Sharma & in the policy document it is Shri

Manmohan Lal Sharma which is correct since both names are of the same

person.

As I mentioned in his earlier policies no:20108671905808

&201105807 the names were either MrM.M Sharma or Shri Manmohan Lal

Sharma but the death claims were settled very swiftlywhich can be

verified by checking the records.

I request your office to look in this matter seriously & get me

the claim benifit quickly.
