Non receipt of cheque book and atm card ag a/c no. 002101044862

Dear Sir,

Please note that I have given the request since long to your bank to issue me cheque book and ATM card but sorry to say that till date nothing has been received. Now i am in receipt of your letter No. I/2312 dated 30.01.2009 that my a/c no. 002101044862 has been became inactive. which is totaly mistake of your bank.

Now I request you to please give me new cheque Book and atm card on the following address : -

Please note that above a/c No. 002101044862 has been opened during 2004 when I have been workign with M/s Kadimi Tools which I have been left but my you have to send the above cheque book atleast my home address but there I have also not received.

Please send me cheque book and atm card on my office address.
