Delay of ATM Pin & Card

Dear Sir,

I want to inform you that I shaheen Fatima has applied for ATM from the HDFC Lucknow, Alambagh branch through the executive of Mr. Kuldeep. I had given him filled form with all required document along with Cheque of Rs. 5500/- in April. The amount of give Cheque (Rs. 5500/-) had been deducted by the HDFC on 22nd April'08 from my account, but I did not get any pin No. or any information regarding the ATM. Mr. kuldeep is not receiving my phone and I had complaint directly many times in Alambagh branch but no body looking after this matter. There fore I requesting you kindly solve this matter as soon as possible so that I may take ATM or money. Hope your cooperation will help me.
