Non issue of duplicate pan card

I lost the Pan Card while travelling in Mumbai alongwith some otherdocuments. I reported the matter at Bandra Police Station and filed a FIR. I applied for a duplicate Pan Card on 11th May through your centre at Ernakulam, MG Road. The coupon No. is 033407188 dated 11-05. My application for New Pan Card vide coupon No. 033407188/ 311249065/ 704398571 for PAN AOPPR2264A

I Invite your attention to your above referred letter. In this connection I have to the correct name of my father has to be read as TRIPRAYAR KRISHNAN RAMAKRISHNAN, which was erroneously mentioned as TRIPRAYAR KRISHNAIYER RAMAKRISHNAN as per my original application. I enclose the copy of the PAN card No. ACNPR5477K of my father in this regard.

2. I also forward herewith the copy of my original PAN CARD AOPPR2264A which was lost when my personal belongings were snatched at Mumbai.

3. I also enclose copy of the relevant page of my SSLC Book as my residential proof.

I request you to forward the dup0licate PAN card at the earliest.
