Dear sir,

I have applied for change in PAN CARD. On 22nd may I received a sms, that your request has been approved by ITD & the PAN CARD will be dispatched shortly. But I did not receive it. On enquiery I came to know that the card has been deliverd to some priya kapoor at, PITHAMPUR, instead of my KHANDWA address on 28th may at 15.15 hrs.

I request you to kindly enquire into the matter and instruct the concerned person to send it to my Khandwa address.
