Mistake in Name

I have a policy of my son Swarnabh Ghosh, Policy No. 115532329 where the name is wrong. At “Proposal Deposit Receipt” the name is right where it is written as “Swarnabh Ghosh” but the receipt I get and the Policy Paper you send to me, there I found the name written as “Jishnu” which is wrong. “Jishnu” is the nick name of “Swarnabh Ghosh”. I was surprised how it is possible where I also submitted the Birth Certificate of my son where it is clearly written the name of baby is “Swarnabh Ghosh”

Therefore, I request you, to kindly change the name and also send me a fresh receipt and Policy for the same.
