Sub: Passport Not Received.
Respected Sir,
I am Uday Bhaskar Uppala, applied for the Passport in the year 2007 from my permanent address Vijayawada with a File No—A308733-07 on 18/Oct. At the time when I applied for the passport, I was pursuing my Professional Degree MBA, at “Magnus School of Business” in Hyderabad. I had mentioned my college address as my present address in the passport application form. The police enquiry was done at my permanent address, Vijayawada. But due to some mis-communication the police enquiry has not done at present address Hyderabad. So I have received a letter from the passport office stating that “I am not residing at the present address”.
I had submitted my PPF forms with the required documents in the passport office for several times. I have also attended “LOKE ADDALAT” to get my passport, but I have found no improvement in my case. I came to know that after being completion of one year we can again apply for fresh passport. So, I have applied again for fresh passport on 3/Oct, with a File No—A307056-08, stating my permanent and present address as Vijayawada in the application form. This time also the police Enquiry was done, but I failed to get my Passport.
I was not given information of applying for the passport for the second time with the previous “File No”. With out having proper information I have applied for the Second time. I was neither received “First” nor “Second” passport.
I have also paid the fine of Rs. 1000/- on 8-April-09, for re-applying the passport. But still now there is no action from your end and was not updated in the passport status page, same mess was being displayed since last 2 months.
So, kindly take the necessary action and please close my issue as soon as possible.

Present Status& Application has been closed, Please reapply quoting earlier File Number or contact PRO.
2nd File No—A307056-08. Applied Date: - 3/Oct.
Present status--FURNISH EXPLANATION FOR SUPPRESSION OF MATERIAL INFORMATION ABOUT THE PREVIOUS PASSPORT AND SURRENDER THE SAME.
