Correction of husband name and state not mentioned

Sir,

this is in regard with the correction at husband status. in my previous passport about my husband name it was late V Srinivasulu but in the renewed passport it was appearing as V Srinivasulu. and state ANDHRA PRADESH to be mentioned at possport. pl guide me for this rectification.
