Harrrasment & failing to atttend request

Dear sir

i made following request

name change from Sampan sharma to shailender sharm (between husband & wife)

address change-my office adress mentioned in the form

mob. no -9810588828

name change from shailender sharma to Sampan sharma (between husband & wife)

address change-mayur vihar address

plan change- from 150 freedom to 'get more plan'

i submitted request on 6th december with all neccessary documnets & paid billed as well unbilled amount.

even after person visits so many times nothing has been done so far.

although i was contacted on pohone for confirmation of details, no progress yet.

now i am planning to cut off from airtel & move to consumer court
