I have received my VOTER ID Card No. “LHV3060712”, but there is no sur name indicated in my voter ID.!!

My and my Father's name in english is also incorrect and wrong spelled and also my birth date is wrong mentioned.

WRONG INFO MENTION IN THE CARD

Name: BALARAM

Father's Name: SANTASHARAN

Birth of Date: XX/XX/1987

CORRECT INFO THAT I NEEDED ON MY CARD NO. “LHV3060712” :

Name: BALRAM SONI

Father's Name: SANT SHARAN SONI

Birth of Date: 10/04/1980

What i should do to get correctly printed all this info on my voter id.
