With reference to the captioned subject, I wish to bring to your kind notice that on my resignation from ACC Ltd., I have forwarded duly filled in form 10-C during July (i.e. Annexure-4) for employers attestation and submission of form 5 to Gulbarga RPFC office. (i.e. Form 10C (EPS) - Claiming withdrawal benefit - KN/3747/3722).
Inspite of several reminders and follow-up's no action was taken to process accumulation in EPS contributions. Keeping in my mind the long pending issue, I would urge your good office to direct FP commissionerate to process my legitimate benefits.
