My new sim is blocked

Hello Sir,

My new sim which i had purchased from a dealer registered under my name of M G Umesh s/o M N Gangappa my date of birth 04/12/1979 has been blocked while i was in process to make security code for the sim. The mobile no is 7376346360 and the sim no is 89915 52036 12577 7908. well this info is enough to unblock my sim.
