Atm card not received yet

Dear sir/madam,

this is krishna thoodi, acount holder of state bank of hyderabad, sulthanabad, karimnagar dist, since my ac no is 62046544049 (savings). my old atm card had broken because of this reason i have applied for new atm card at my home branch 3months back that is on 24/04and deposited Rs 200 towards new atm card charges.but i have not received atm card untill now.i have consulted the branch one month back and the branch manager has told me apply for one more time.i was applied once again but not received my card even after one month.and i have given a phone call to sbi atm card enquiry centre, and the sbi people told your application was rejected.the same was told one month back also.please take proper action on that branch people.
