Notes get CUT by ATM Machine

Dear Sir

I withdraw the cash from the ATM ID: STCC8664 time: 07:58am DT:03-Jun-12 Record No: 8987 for Rs.15000/- from my Account#074010100274012, one of the Rs.500 note which has come is cut by ATM and come outside to me.

The 500 note Serial Number:8LU 525763, request you to kindly guide me to whom should i contact to get the replacement Rs.500.

Please find the scanned copy of the Note which i received from ATM, the Phone Banking service executive says i need to contact the AXIS Branch phone number mentioned in the ATM but nothing is mentioned in ATM room.
