Non receipt of my reward gift

Dear sir, myself sarat kumar nayak, an account holder of axis bank, berhampur, orissa branch bearing ac no 212010100004008. There was an offer declared by axis bank that those who will purchase an amount of 80000 inr from 10th october to 31st nov in debit card will be rewarded a silver gift of 30gm. i have qualified but till now i could not get. i have also given an apllication in branch as well as i also informed 3 times to call centre. no results and response get.
