Debit card blocked

Dear sir,

myself pratibha singh sisodia while transacting online my debit card got blocked due to password information which i always ignore and go through my ATM pin but this time they have blocked and every time i create new password which i cant access next time as it always show invalid i request bank to allot me proper password and unblock my card as soon as possible..my account details are:-
