Delay cheque book

This has reference to my saving account No. 13462 with bank of india i had given request for issue of cheque book at bank of india hinganghat branch on dated 22.08.2011 but till to date i have not recd.chq.book On contacting the concern branch relevant authority they started passing the balls head office. I email to head office at nagpur & hgt.branch but no action has been taken.branch manager are helpless about this matter.
Who should be resposible for all this?
