Less amount withdrawal from ATM

I have withdrawn Rs. 5000/- from the ATM at mahananda nagar on21-05 Tr Id CWDR//983003/CUJ9001. But I could get only 4500/- Rs from the machine. There was no any facility to get receipt from machine. I complained the next very day to the branch but no action has been taken so far to refund the amount. I have contacted personally so many times to the bank but it was told me that the action is due from the Bombay office.
