My debit card is deactivated

I am having a pension account with Bank of Maharashtra, Bhusawal branch 425201. My account No. is 2008311233-5. I am holding a debit card No. 421409014802XXXX.

I had made my last withdrawal of Rs. 3000.00 on 29/05. Yesterday on 01/06 when I tried to withdraw my pension through bank ATM itself TXN No. 7984 and 7985 I received a message "SORRY UNABLE TO PROCESS", RESPONSE CODE 072. I made another attempt through AXIS bank ATM and I received a message TRANSACTION DECLINED 070. CONTACT BANK.

Obviously my debit card is blocked. Please help me out of my predicament by reactivating my debit card and by ensuring that I do not suffer similar hardship again. Thanks.
