Dear Sir,

Myself Amarjeet Karn, my company has medical insurance with paramount health services/bharti axa.

My PHS ID is 11606803 (group id). I have submitted all documents regarding delivery of my wife but insurance company not accepting it. Insurance company demanding from me hospital registration number/number of beds and OT facilities document.

How I can give this document while this is not written in claim settlement procedure.
