Not issuing of atm card

After three years later and filling 10 times ATM application form samastipur canara bank branch did not issue my ATM card. I am maintaining sufficient balance in my account. My account is currently in operating mode. Whenever i visit in branch ATM officer says to fill another ATM application form. ATM officer and Branch manager did not response well.

I want my atm card quickly.please ensure my consumer right and help to provide my atm card.
