Erase my name from cibil

I was holding a credit card of your bank for which i have made a settlement in the month of aug through cheques of which i have sent you the attachments of payment slips earlier but no results from your end. Now it is appearing in cibil when i have appraoched for loan in the last month. Kindly erase my name from the CIBIL so that i can apprach for loans. Also I have tried to take the cibil report from CIBIL WEBSITE but is it asking for the control number which you have to provide me. Kindly provide me the control number inorder to access my cibil report.
