Non issuing Income Tax deducted Certificate (home loan) Under 80C

I Archana Basu have taken a Home loan from Allahabd Uttar Raipur Branch (Budge Budge)... I am facing tremendous problem while i asked for Provisional Statement for Claiming Deductions Under Sections 24 (b) & 80c (2) of the Income Tax Act, 1961. All my colegues have submitted the same with the help of their bank and got deduction in IT... but repetedlty requesed i didn't get the same from my bank .. The bank manager's statement was like we didn't issue such kinda form... And also never he been co-operate to his customer for any other issue... i have to request him like anything to get the deduction from my office... please help me... I know other branches of the same bank issues this form... why I am suffering for this Branch Manager arrogancy? please help me and take a strong action..
