Damage of Debit Card

Dear Sir,

I have a SB account No 11241747230 with SBI, Anaimalai -2194 and the banker has issued a ATM-CUM-DEBIT CARD NO 6220180219400002628 valid from Nov. Due to frequently using of debit card, the card comes to damage for last 15 days and card unable to use. Hence you are requested to issue a new card to my address as mentioned below :-
