On 12/05/201 i forwarded the application requesting for unclaimed divident and adderess change through couriour to, the h d f c bank ltd, legal and secretiral deptt.kamla mills compund, process house 2nd floor, sanapati bapat road lower parel mumbai.my folio no is HB 481940 certificate no.706187, distinctive no.1893316601 to 189316700 (100) shares (COPY FORWARDED).i also stated in my application that in case any formalities are to be completed for this kindly advise me the same .but till to day i have not received any responce from them.
