It is brought to your kind notice that my policy no.12239224 was started from 22.10.08 & the undersigned has deposited regular 3 yearly, instalment i.e. payment of Rs. 15000/- each year total Rs.45000/-

It is also brought to your kind notice that the agent of your concern has printout that yearly payment of policy 12239224 could be submitted for regular 3 year after regular 3 year payment may policy was discontinued and the payment of Rs.22352/- has been done your concern in the name of my wife Smt. Shushma Kushwah as a nominee without my consent and without any application for saranding of policy in the name of my wife.

Now therefore it is to intimate me why the payment has been done without my consent & without any application for my policy.

Now I request to you to take action to the mater & proper payment could be done to me within a 15 days from the respected of this letter otherwise it will be liable to compliant to this mater "Consumer From" this may please be noted.
