Wrongly issued credit card

I have been missguided by the HDFC bank staff, kharghar branch for applying for credit card who has taken my signature on application form no: PB0021 8572 in july-09 month for maiking credit card on my name with promise thatcard will be delivered within 10 days. I requested her to give me photo copy of the aplication form after filling my detail and brfore processing the form. After 10-12 days when i visited the bank, she could not provide me the copy of the form as promised and said that the card delivery will be delayed for a month. As I had to shift my house following week, I written an application for cancellation of my card on grouds that none of her promise was comming true. the cancellation application letter xerox copy with bank stamp is with me. now after 3-4 months i have started getting calls from chennai and hydrabad offices of HDFC credit card that if i have recived my card or not? on contacting kharghar branch the concern person mr Sunil Pillai whom i have given the application for cancellation, he has provided me the ref: no: of my card issued as :09070620668190W1 but was unable to clarify why my cancellation application has not been processed. now eveyday i am getting 2-3 calls from chennai and hydrabad if have recived my card which probably has returened to chennai office as i have shifted my house.

none of the person mentioned above is interested in closing the matter by confirming me that my card is destroyed or cancelled.

it is evry day harasment for me to reply them and request them for solution but no clear cut way out is given by them. i wonder if my card is missused by some one, how to safe gaurd that?
