Subject: Regarding raising dispute for a transactions on my card. (Service Request Number: SR140034386)

Dear Sir/Madam,

I have been issued ICICI Titanium Card: XXXX XXXX XXXX 8018 in 2007. Nobody else has access to the card.

I have never provided card details to anyone on phone or internet. Thus there are no chances of getting the card details such as card number, CVV number,

and expiry date etc.

On 30/04 there has been a transactions made to the Merchant Name "GARP" for USD 150.00

The above transaction was done by me or authorized by me.

I called up the call center as soon as I realized foul play and blocked the card and was asked by the customer care representative to send a fax with the dispute information. I have already sent the fax to the number provided by the customer care rep which is 02266466683 .

Please investigate this dispute and provide me with a written statement of the outcome. Thank you for your time and attention to this matter.
