PF not received

Dear Sir/Madam,

With due respect I would like to draw your attention towards my claim request for Provident Fund. I was working with ICICI Bank, Delhi. My PF No. is MH/18796/139235.I have sent my PF settlement claim form, duly filled in, to the following address on 14th December.

ICICI BANK Ltd. Provident Fund

Laxmi Commercial Complex,

2nd Floor, C Wing,

Behind Nabard Bank,

Bandra Kurla Complex,

Bandra East, Mumbai-4000 051.

I have neither received any settlement amount nor any communication till now. While checking with EPFO office they are saying that only ICICI Bank will credit it as it is an exempt organisation.

ICICI Bank HR is not responding. Apart from my PF, my one and half month salary and PL leaves compensation is already pending with ICICI Bank.

Please guide me on how to get the PF status and whom to contact.
