Cheq book not resived

Dear sir,

I Mhammad. Minhajuddin A/C holder with A/C No:-037001512639' at Nallzkunta branch, I plased a cheque book request through phonebanking on 11th and one of the executive asked me call back on16th to collect the courier shipment number, I got the shipment no 4385802 and cont no 65908183 of prvs courier, Since 16th iam continuesly calling them but there is no proper responce.we as customer strongly belive in good service by icici bank.it is very disappointing for me that icici bank is fail to giving proper service.
