Regarding fixed deposit payment

I, 'Smt. Vasudha Waman Datar', had a fixed deposit with the Sangli Bank (Now merged with ICICI bank, Lakshmipuri Branch, Kolhapur) which had got matured in year 2000.In year I had submitted the certificate to issue the corresponding cash, but now the staff was refusing to give back the same. I have the receipt which tells about the submission of the certificate.
