Reference: Policy No. 07388626 – Bhikhabhai K. Patil

With reference to my earlier letter dated 23.06.09 & 15.09.09 to ICICI Prudential Life Insurance Co., Naranpura Branch, Ahmedabad regarding non credit of my premium. I have to inform you that I have submitted a cheque and notice for premium receiving from you. In hurry I have wrongly mentioned Policy No. 07388628 instead of 07388626 and I had also written my name Bhikhabhai K. Patil on the backside of cheque No. of 947952 Dated 10.01.2009 of Bank of Baroda, Kaligam branch is already debited from my account for Rs. 20, 000/- on 20.01.2009 and photocopy of passbook was already submitted.

I was given assurance that your account of policy will be credited soon and premium receipt will be issued. But till date I have not received any receipt of the above.

Therefore, please rectify the error and arrange to issuance of receipt for premium and also I like to inform you that, please take necessary action for the same and do the needful.
