No full and final no P.F

Respected sir/madam,

My name is Sidharth Jain and my employ.code-FOS5773. I was sales executive (SE3) in Ur bank from 25 may at Ferozpur Road (271) branch in Ludhiana from 18 Oct. but my appointment letter comes on 31 July .i left the job due to some personal reasons. My branch manager is Mr. Sanjay kaul at that time. i haven't got my last salary for the period of 1 1/2 months. I want to know my P.F status as its cut from my salary . please help me in this matter.
