No reply against request for issue of cheque in the name of nomine

The policy no 520446930 of jeevan surksha, pension plan scheme was i n the name of my father sri shital prasad. in an unfortunate incident he was short dead by unidentified guname in the wee hours of 12/10/09.

cheque from nov to march was kept uncashed at home and same was returned through LIC agent to zonal office with the request to reissue the cheque in the name of nominee . no reply was recived . again similar letter was send to zonal office patna thorugh branch office in march with the request to isse the cheque in the name of my mother smt kamal sinha.

no reply has been issued till date.

therefore it is requested that necessary action in this regard may please be initaited at earliest.

the nominee is alone at home and there are not enough people around to help her.

your early attention will be valuable help to the family.
