I am a holder of Jivandhara single premium policy no830218871 Premium Rs 25170 Receipt dt 31-3-93 Branch 5/842 Premium paid on 28-3-93 This policy is matured on 1-4 and therefore annuity instament dues on April. Inspite of my several personal visits to your office at AHD & several tele talks with your officials I have not started receiving Annuity payment. More than a year have lapsed but all in vain.name of the officials are Mr Suresh Patel, Madem Zarana Mr Pareshbhai and lastly Madem Kalindibahen onApril 20.
