Dear sir/mam

Pl. refer to the contract note no. N/NN/231/756975 issued for my trades under trading ID-N122322 . This is to inform you that I have not done any trading in the shares of DLF on 11/12 but there are many trades appearing in my contract note. Pl. look into the matter and confirm why these trades are appearing in my account and issue me a rectified note immediately.

One occasion earlier also some unathorised trades were done in my trading ID which were also telephonically conveyed, but till date not action has been taken.

Pl. not that in the absence of any corrective action from your end, I will take up the matter further to NSE & SEBI.
