Respected Sir, With due regards it is to submit that I had already left my job and submitted my claims form 10C claim ID: KDMAL110900005514 for withdrawal benefits/scheme certificate members A/c No. KDMAL00384910000004494 in MAY. Sir my claim ID is KDMAL110900005512 status claim form No.19 for PF settlement A/c No. KDMAL/PF/3891/4494. Sir this had been received on 15-09 and shows "UNDER PROCESS" till date. Accordingly, I have not received my final settlement claim till date. Sir I am facing the financial hardship these days. It is therefore requested that my both the claims may please be settled as early as possible. I will be ever thankful to you for your kind help in this connection.
