Onus share and unclaimed dividend not received till date

DEAR SIR/MADAM,

I, Asha Kedia beg to apprise you that i was holding physical shares of kwality diary under folio no 011378 and recently dematerialsed the shares into my demat account no IN 302978-10254299

But I am sorry to inform you that after several written and verbal communications with their Registrar, my due bonus shares has not been credited to above mentioned demat account nor physical certificate has been sent to me by them and nor i have recived my unclaimed dividend till date.

I therefore request you to look into the matter awaiting for your early response in this regard.
