Request for Pending Provident fund

Dear sir,

This is yaswanthkumar from Nellore Andhrapradesh. Iam

working as a sales executive in SBI Credit Cards from the franchise of

M/s Lakshmi Enterprices, d.no.16-82, upsatirs of sireesha

nethralaya, pogathota, nellore-524001.From the period of 05-05 to

10-07.That time Iam asking to manager he is said after completing

3months. Iam going after three months that office is closed. That

propriter name is m.r.balamurali, he will back after closing six months

in nellore he had signed all members and he say i will pay all your pf

amount but that dat to this day he is not contact and he also chenge

is mobile no. In that frnchise working total 35members but any one not

gain that pf. My provident fund no is ap-50449-24, also iam sending my

providentform 2times in cuddapah office but it is regected because

that form not signed the propriter iam told the providentoffice his

suggition please sign any bank employee after send your provident form

also iam sending but it is not consider. So please verify the Details
