Dear Sir/Madam,

One of my customer, Mrs Jeliben M Patel, has taken the LIC Policy vide policy number 862737782, I have not received one of the annuity payment cheque for which I have written several letters to LIC consumer cell in Ahmedabad but have not got any revert from them. She is frustrated with this kind of unprofessional behaviour from side and would like to know the status of my cheque no 529357 for amount Rs 24, 000 dated March 01, If the same is unpaid kindly send me the duplicate warrant on priority or give me the solution for procedure how to apply for duplicate warrant
