REMINDER - IV

Not getting Payment of annuities of my Policy No. 482101004

in my SBI bank Account No. 10432963228 from October onwards and it is going to be more than a year. I had already send Speed post and Registered letter to your office twice and twice I had complaint on Internet but till now I am not getting any information regarding this matter even I had submitted ECS/NEFT mandate form duly signed and attested by bank along with one blank cheque as per required by your office. If there is any formalities left kindly give me information as early as possible. Again I hereby remind you that kindly take seriously; otherwise I have to take legal action.
