Dear Mam/Sir,

I am very very sorry to write you as wel as lodging a written complain that survival benefit amount Cheque against my Policy No.-12276641, retained by agent (code-00250320) till 12.02.2010.

Previously, Agent (CODE-00250320) in Nov., failed to inform/despatch me. And never informed me.that time, there was no another sources to continue my wife medicine & to my urgent Tibia & Shoulder Fracture Accident.

I was Hopeful on This amount.

Although, I got Major Accident in Nov. & need money to operate, and My wife is Patient of Lupus Nephritis and take costly medicine per month. That time I was complete Bed.

when My hand Started movement, Then, I Written to LIC marketing Head Office, They inquire Positively and Today Branch Person Ms. Priti G, call back to me & inform me, branch has given/informed to cheque to your Agent, but, he didn't inform you.so, today I am canceling the Previous Cheque and sending a New Cheque, who will dispatch on Monday.

I agreed with Branch, But the My Complain is-

1. Why Agent N't send/Inform to me from last 4-5 months.

2. Who will pay Compensation against my huge loss.

3. who is Responsible for delay in my operation (because of accident, TIBIA FRACTURE) and my Wife wife monthly medicine.

4. Why Previous cheque (NO.685795, DATED-24.10.10) Retained by Agent,

uncessary.

I will Provide Treatment Copies, when required.

Every Person Take Money Back Policy according to Future need of survival benefits.

Please take necessary legal action (Immediate Cancel registration)& stop Agent Comission .take serious Action against this type of Irresponsible Agent, Which Impact Bad effect of reputated LIC to customers in this Competition time.

Failing This I will move for Legal & aware to Media.
