Dear sir,

It is to inform you that i am a policy holder of policy no (570768991) opening Dt;26/02.At that time i was working as Assistant District Welfare officer in Sub-collector office, koraput (orissa). I have been retired from Govt.service w.e.f 30/11 and residing in my native place in the following adress. I have already submited my enclosement details to IPPCELL, JEEVANDEEPBUILDING, EXIBITIONROAD, PATNA.But i have not been recieved the mail or annuity till date.
