CORRECTION IN NAME

SIR,

My policy NO 874186895, in which my name has been mentioned as PRASHNATKUMAR SHANKARBHAI PADHIYAR . where as my name is PRASHANTKUMAR SHANKARBHAI PADHIYAR, so far while receiveing the premium notice I use to consider as typing error, but when I checked my policy the matter is same so now what shall I do to get it changed to (PRASHANTKUMAR SHANKARBHAI PADHIYAR). Kindly advice me what to do, I can send you the copies of my pan card / I - card / licence. whichever is necessary . my E-mail ID is prashantpadhiyar@yahoo.com. hope an early reply.
