Dear Sir,

Sub: Request for payment of maturity value of FDR No. F/0698029620/1 dated September 6 1998 date of maturity 06/09/1999 under MTHL Scheme.

With reference to the above I would like to draw your kind attention that I have received a sum of Rs5000/- being 20% of the face value of the FDR of Rs25000/- and rest Rs20000/- is yet to be paid to me. The original Deposit Receipt has been submitted to you on 13/08 at your Patna office (a photo copy of the duly receipted Deposit Receipt is enclosed herewith for your ready reference and record) for the purpose.

In this connection I would further like to draw your preferred attention that I am a 65 years old man with various old age complications and my wife also remains sick. I have no regular income and major part of my help from my only son goes on medical expenses.

I would also like to mention you that for better medical facilities I have finally shifted to Kolkata at address below and request you to send payment/ make necessary correspondences at new address only.
