Pf withdrawal

Dear Concern,

My name is Deepika Dhiman & I worked for mafoi (35791) as deputee from

Sep'08 to Oct'09 in ICICI Prudential Life Insurance Co. Ltd. For which i

put my PF withdrawal (PF no.135383) request in the m/o Nov'09 in local mafoi office.

After so many reminder i am unable to get the information of my PF

withdrawal request. Now the PF is credited in my account but my pension

scheme amount is not credit, but in urs records seems that its credited in my account. Account no is 001701554172 with ICICI Bank ludhiana Punjab. Pls look in to this matter. I m sending so many reminders to PF department but there is no response from PF department.

So., U are requested to personally look in to this matter & do the

needful ASAP.
