Purchased Shares not Transfered and contract note is not provided

I was purchased some shares and payment has been paid by Cheques which was Debited in my bank account but not credited in my D Mat Account and no contract note was provided to me. Some Shares (12 companies) was transferred by me from R.R Brokers to my D Mat account in Mansukh Securities and Brokers but these are also not showing my DMat account. A excel sheet has been provided by Brokers representative which was false as per records.

I want to my All shares may be credited in my D Mat account.

I want to issue me contract note from opening date of my account in BSE/NSE.
